Reimbursement for telemonitoring in heart failure from January 1, 2025

Starting January 1, 2025, reimbursement will be available for telemonitoring follow-up of patients recently hospitalized for heart failure. This blog provides an overview of what the reimbursement entails, the applicable conditions, and how Remecare can support hospitals in implementation.

What does the reimbursement cover?

• Initial forfait: €200 for the first month.
• Follow-up forfait for months 2 to 6: €90 per month for monitoring weight, heart rate, and blood pressure.
• Follow-up forfait from month 7: €45 per month for the same parameters.

Who is eligible for telemonitoring follow-up?
Reimbursement is intended for patients who:

• Have been hospitalized for heart failure, or
• Had a complicated hospital admission due to heart failure.

After stabilization, these patients must be classified as NYHA II, III, or IV.

Telemonitoring team composition
The telemonitoring team consists of at least:

• A cardiology specialist,
• A specialized nurse.

This team collaborates closely with the patient’s general practitioner, who manages the global medical file, ensuring optimal coordination and follow-up care at home.
